You will need to bring valid photo ID and have the physical address, mailing address and phone number.

If you are a property owner or property manager you need to complete and submit a Property Owner Utility Services Request. Valid government issued photo ID is required. If you are requesting services remotely the form will need to notarized. The original notarized form is required, no photo copies.

If you are a tenant, you will need to complete and submit a Tenant Utility Services Request (completed by tenant) and a Utility Services Waiver (completed by property owner or manager).
